Gonarthrosis

pain from arthrosis of the knee joint

Most often, the disease occurs in older patients (especially women).The joint pain associated with gonarthrosis is explained by dystrophic changes in the cartilage covering the intra-articular surface of the bones.First of all, the blood circulation in the blood vessels feeding the cartilage deteriorates.Then the cartilaginous covering begins to thin and is completely erased.The bones inside the knee joint become exposed.As a result, they rub against each other while walking, causing severe pain.

Over the years, bony growths begin to appear in the joint.They trigger the development of deformity, the leg bends.

Causes of gonarthrosis

Gonarthrosis of the knee joint occurs for various reasons.Two forms are usually distinguished:

  • primary;
  • secondary.

Primary gonarthrosis usually develops in older people.Cause of occurrence:

  • metabolic disorders;
  • obesity;
  • hereditary predisposition;
  • taking hormonal drugs.

The primary form affects two knee joints at the same time and is called bilateral gonarthrosis.But there are cases when the inflammatory process first spreads to one knee, and only a few years later it moves to the second.

Secondary gonarthrosis, on the contrary, is always unilateral.It is called:

  • heavy physical work and sports that stress the right or left knee joint;
  • trauma (ligament rupture, dislocation, bleeding into the cavity of the knee joint, damage to the meniscus, intra-articular fracture, etc.).
  • operations on the knee joint;
  • inflammation/swelling of the joint.

Symptoms of gonarthrosis

Common symptoms of gonarthrosis of the knee joints are as follows:

  • severe pain in the knee during movement;
  • knee dysfunction;
  • swelling in the knee joint;
  • lameness;
  • joint stiffness;
  • instability in the affected joint.

The late stages of the disease are also characterized by deformation (curvature) of the foot.

In most cases, gonarthrosis develops slowly and passes through four consecutive stages.

First-degree gonarthrosis of the knee joint is characterized by the following symptoms:

  • Undefined pain in the knee when going up/down stairs, while squatting.
  • "Starting" pain when starting to move.
  • Restriction of mobility.
  • Disappearance of discomfort when the patient "diverges".
  • Knee pain after heavy physical activity.
  • The knee appears healthy or slightly swollen.
  • Manifestations of synovitis (inflammation of the inner mucous membrane, resulting in fluid formation in the joint) may occur.

Second-degree bilateral gonarthrosis of the knee joint has the following symptoms:

  • The pain becomes more intense.
  • Discomfort increases sharply when squatting and walking.
  • The pain is localized on the inside of the knee and gradually disappears during rest.
  • Crackling in the knees.
  • The joint expands and fluid begins to accumulate in it (synovitis).
  • Fluid within the joint can fill the popliteal cavity at the back of the joint (Baker's cyst).
  • Some joint areas are hot to the touch.
  • After a physical warm-up, the stiffness may disappear.
  • The shape of the legs changes in the area of the knees (curvature is observed).

The following symptoms are characteristic of third-degree gonarthrosis:

  • The pain occurs day and night, while resting and walking.The patient should seek a position that reduces the severity of the pain symptom.
  • The joint is difficult to straighten and bend.Limited in movement.The leg cannot be fully straightened.
  • An O- or X-shaped deformity may develop on the lower limbs.
  • The size of the knee joint is significantly increased and deformed.Severe synovitis is observed.
  • The patient's gait becomes unstable and shaky, and he must use a stick/crutches or resort to external assistance when moving.

There is also a fourth degree of gonarthrosis of the knee joints.Clinically, orthopedists classify it in the third stage of the disease.This can only be determined with an X-ray.

In the fourth degree, the joint surfaces of the bones are so severely deformed that there is almost no gap between them, without which movement is impossible.The patient suffers from constant pain.

If you notice similar symptoms, consult a doctor immediately.It is easier to prevent a disease than to treat its consequences.

Diagnosis of gonarthrosis of the knee joints

Diagnosing the disease includes interviewing the patient, examining the inflamed joint, checking its volume and assessing mobility.If gonarthrosis is suspected, the orthopedist prescribes instrumental methods:

  • x-ray.In the case of gonarthrosis, the X-ray shows the narrowing of the cracks, the thinning of the bone tissue and compaction under the cartilage, and the ossification of the articular cartilage.They can be osteophytes - bone growth running along the edges of the cartilage.
  • Ultrasound diagnostics of the knee joint.
  • Arthroscopy.During the examination, the surgeon inserts a special micro-video camera connected to an arthroscope into the joint through punctures.An image of every part of the examined area is displayed on the screen.
  • Magnetic resonance imaging (MRI).It allows the assessment of the degree of damage to the knee joints.
  • A blood test is an additional diagnostic method and does not always give a complete picture of the clinical picture.The fact is that in the case of gonarthrosis, signs of inflammation are not detected in the blood.

Treatment of gonarthrosis of the knee joints

Medicines

The following drug groups are used in the treatment of gonarthrosis of the knee joint:

  • anti-inflammatory;
  • pain relievers;
  • hondoprotectors (they ensure the restoration of damaged cartilage tissue).

They are available in various forms - ointments, creams, gels, tablets, capsules, intra-articular and intramuscular injections.

Orthopedics

Orthopedic treatment of the pathology involves the use of special devices to minimize the load on the diseased joints:

  • orthopedic insoles for shoes,
  • orthoses (special rigid knee pads that limit the mobility of the affected joint),
  • commas.

Surgery

Surgical methods are used in the treatment of third degree gonarthrosis of the knee joint.Indications for endoprostheses are as follows:

  • uncontrollable pain
  • significant dysfunction of the knee and hip joints.

The time spent on implantation is 2-3 hours.Before inserting the prosthesis, the damaged part of the joint and the back of the patella are removed and the axis of the leg is aligned.If necessary, we will restore the tapes.When installing the endoprosthesis, the wound is sutured.

In the first hours after the operation, the patient does not feel the lower limbs due to anesthesia.The rehabilitation period is long - 3-4 months.

Endoprosthesis surgery is impossible if the patient suffers from osteoporosis (fragility and fragility of the bones).

Additional methods of treating gonarthrosis

In the case of gonarthrosis, the following can be used as additional treatment methods:

  • manual therapy;
  • massage;
  • acupuncture;
  • hirudotherapy;
  • physiotherapy;
  • Exercise therapy.

Practical therapy for gonarthrosis

Exercises for gonarthrosis of the knee joint are selected individually.Their regular implementation makes it possible to strengthen the muscles surrounding the inflamed joint and improve blood circulation in the area of pathology.These should be performed lying on your back or sitting.

In the first days, the therapeutic practice of gonarthrosis of the knee joint takes no more than 10 minutes.Gradually, the training time should be increased to half an hour.It is important to perform each movement smoothly and ensure that the pain does not increase.

Universal exercises include:

  • Raise the straightened leg from the floor to a height of 15-20 cm and hold it in this position for the maximum time.
  • Raising the straightened leg to a height of 15-20 cm and gradually lowering it to the starting position.
  • Bending and stretching the legs while sitting on a chair.

Any squatting and long walks are contraindicated without prior consultation with an orthopedist.

Features of treatment of gonarthrosis depending on the degree of the disease

Treatment of gonarthrosis of the knee joint of the first degree

The first stage of the disease is the mildest.The patient is prescribed:

  • Massage.
  • Therapeutic gymnastics.
  • Physiotherapy procedures.

If you are obese, you need to lose weight.For this, a special diet must be followed.

Treatment of secondary gonarthrosis of the knee joint

Complex treatment of pathology includes:

  • Performing the exercises recommended by the therapist.
  • Taking anti-inflammatory and pain relievers, chondroprotectors, injecting hyaluronic acid into the joint.
  • Phytotherapy procedures (if there is no pronounced inflammatory process).
  • Weight normalization.

Treatment of third-degree gonarthrosis

The therapeutic treatment of third-degree pathology is similar to the measures used in the second stage of the disease.However, in the absence of the expected effect, the patient must use a surgical method.

If we talk about the beginning of the third degree, it is possible to remove part of the damaged cartilage and bone growths.The doctor uses an arthroscope to examine the cavity of the knee joint and assess the severity of the problem.

Corrective osteotomy is recommended when areas of healthy tissue are preserved.During the operation, the doctor corrects the shape of the bone by breaking it.He then installs a special plate that allows the limb to be fixed in the correct position.

The second method of surgical treatment is endoprosthesis - an operation to replace the entire knee joint or some of its parts.

Treatment of gonarthrosis of the knee joints with folk remedies

Traditional recipes help to achieve good results only in the 1st and 2nd stages of the disease.When the cartilage tissue is destroyed and the joint is severely deformed, they are helpless.

Ficus tincture

The solution is prepared according to the following recipe:

  • Pass the ficus leaf through a meat grinder.
  • Place in a glass container.
  • Add 100 ml of vodka.
  • Cover with a lid and leave for 3 weeks.
  • Store the prepared liquid in the refrigerator.Rub the tincture on the joints once a day for 10 days.

Clay poultices

  • Place the blue/red clay in a glass bowl.
  • Dilute with water to the consistency of sour cream.
  • Apply clay to the canvas/cotton fabric about 1.5-2 cm thick.
  • Apply it to the painful joint.
  • Secure with a bandage and wrap with a towel.
  • Make a poultice 2-3 times a day.The duration of treatment is 1.5 weeks.

Dandelion tincture

  • Fill a bottle with dandelion flowers.
  • Pour triple cologne on top.
  • Close the lid.
  • Put it in a dark place for a month.
  • After the specified time, filter the tincture.Rub your knees twice a day.

Apple cider vinegar for arthrosis of the knee joints

  • Mix 6 tablespoons of apple cider vinegar with 2 tablespoons of natural honey.
  • Apply the mixture to the joint area.
  • Cover the top with cabbage leaves.
  • Wrap in plastic and scarf.
  • It is best to apply the poultice at night.Remove after the leaves are completely dry.The duration of treatment is approximately 4 weeks.

Garlic with celery and lemon

  • Chop 250 g of celery, 120 g of garlic, 3 lemons and mix.Transfer it to a three liter bottle and pour boiling water over it.
  • Cover with a warm blanket.
  • After cooling, use 70 ml before meals, first once a day, after a few days - 2-3 times a day.

Diet for gonarthrosis

The diet for gonarthrosis of the knee joint is aimed at normalizing body weight, fighting obesity and saturating the body with nutrients.If the patient manages to lose weight, the pain will be less pronounced, as there is less stress on the joints.In this case, you should not starve or resort to a mono-diet.Nutrition should be balanced.

Proteins help restore intra-articular cartilage.The patient can obtain these from lean meats, cheeses, cottage cheese, fish, lentils, beans and buckwheat.Jellied meat made from bone broth is useful for gonarthrosis, as it contains a lot of collagen.

Carbohydrates must be obtained by eating vegetables and fruits.Fats should be predominantly of vegetable origin.A small amount of butter is allowed.Vitamins, especially group B, are of great importance in improving the patient's condition.They are found in eggs, legumes, bananas, dairy products, cabbage, nuts and lentils.

It is forbidden to drink alcohol if you have gonarthrosis of the knee joint.It not only has a negative effect on the body as a whole, but also increases the appetite, which is not desirable.

The danger of gonarthrosis

In the case of first and second degree gonarthrosis, we are not talking about disability.The third stage of the disease can incapacitate the patient, when the functions of the joints are completely lost and unaided movement becomes impossible.

It should be understood that gonarthrosis is a chronic pathology that is constantly developing.A favorable prognosis requires appropriate medication selection and regular exercise.

Risk group of gonarthrosis

The risk group for gonarthrosis includes:

  • professional dancers;
  • athletes;
  • builders, loaders, miners;
  • people with a hereditary predisposition;
  • obese persons;
  • with bad posture;
  • suffered fractures;
  • with developmental disorders of the ligamentous-articular apparatus;
  • those suffering from hormonal, rheumatic and metabolic diseases.

Prevention of gonarthrosis

To reduce the risk of gonarthrosis of the knee joints, the following recommendations should be followed:

  • do physical exercises;
  • avoid excessive physical activity;
  • control weight;
  • ask for qualified medical help in case of injuries, arthritic diseases;
  • do not overcool (if there is a problem, it is recommended to wear warm knee pads);
  • drink 2 liters of water daily;
  • wear comfortable shoes;
  • avoid stressful situations.
